Arthroscopic Meniscectomy

Meniscus is a shock absorber of the Knee. Its wonderful position and shape helps Knee Meniscectomy. We usually conserve the form meniscus if it is repairable and viable. But if the form meniscus is old, nonviable then Arthroscopic Meniscus trimming is done. Meniscus tear can be associated with other ligament injury. Most commonly associated with ACL tear.

ARTHROSCOPIC MICROFRACTURE

Do you have Knee pain due to injury/ sedentary life style at young age. It could be due cartilage.

Micro fracture surgery is an Joint Cartilage repair technique performed by creating small fractures in the underlying bone in order for new fibro-cartilage to form. short term results are good but long term benefits don’t last.

How is it done?

The procedure is done through Keyhole Surgery. Holes are made in the Cartilage defect. Through the progenitor skin cells seep in and fill the defect with the clot. This clot heals and forms a fibro-cartilage. If there is malalignment at the Knee joint causing the overloading of the particular part of Knee. Micro fracture is combined with HTO [High Tibial Osteotomy]. If you have Knee pain and want pain relief by this procedure in India.

OATS / MOSAIC PLASTY

It is done for patients with Joint cartilage injuries of the knee. Best used for lesions between 1cm and 4cm . In this cylinders of various diameters 2.7 to 8.5mm are harvested from the less weight bearing part of the knee. These are called Donor plugs. The injured site is prepared Donor plugs are placed in the recipient site.

These steps are repeated to fill the gap at the recipient site with multiple Donor plugs. The clinical outcomes following OAT have been successful overall compared with preoperative conditions. Lesion size less than 2cm treated using OAT was associated with substantially high rate of return to sports when compared to largerlesions.

Rehablitation consists of

      • Phase 1: Protection & Joint activation
      • Phase 2: Progressive loading& functional joint restoration
      • Phase 3: Activity restoration Progression to full weight bearing occurs between 4 & 6 weeks

Autologous Chondrocyte Implantation (ACI)

ACI is mainly used for the treatment of Medium or large Joint Surface defects/lesions
(2-4cm ) for which other procedure like OAT/Micro fracture are beginning to lose efficacy.

Cartilage biopsy is obtained through Arthroscopy. About 0.5 to 1cm of cartilage from less weight bearing surface is taken and shipped to culture facility

Reimplantation of the newly grown cartilage are ready for reimplantation for 3 to 6 weeks. Associate bone defects should generally be bone grafted concomitantly with
ACI (sandwich ACI).

Rehabitation protocol varies widely depending on the location of the defect.
